Abstract of Phase II Trial Results for Low Dose Naltrexone (LDN) for Crohn's Disease from Dr Jill Smith, Penn State University

Abstract Link

This is a highly significant result for Low Dose Naltrexone as it's a Phase II study which was carried out under randomised, double-blind, placebo-controlled conditions.

Summary -

45% of 40 patients achieved remission inside 12 weeks.

(Note, clinical experience shows it can take more than 3 months to really get results with LDN and certainly 4.5mg is often too high a dose too).

82% of the 40 patients had their Crohn's improve by around 20% or more in terms of its activity.
Healing was reported in the endoscopy studies too.

Although this is not quite as impressive a result as the first study, it is at least as good as many other treatments and with a lot, lot less side effects (Only reversible liver enzyme changes reported).

For comparison, this anti-TNF treatment managed 40% remission after 5 years, chances are LDN could be 5%+ better than that :

Remicade + azathioprine managed roughly 50% remission after 26 weeks, but that is of course at a big price financially and in terms of side-effects -

Obviously a Phase III study will be interesting now as it will have a much bigger patient population and maybe run longer too.
Would be interesting to see how the remission rate is after another 3 months.
Would be even more interesting to see how it was if the patients were allowed to decrease dose too.

Note that Dr Smith has another study starting on Vitamin D with Crohn's too.

All drugs need to be thoroughly tested to make sure they are safe enough to be made available to patients. Most research of this kind is done by the pharmaceutical industry. The Government is aware that occasionally there will be few or no commercial incentives for companies to fund clinical trials.

The Department of Health’s National Institute for Health Research (NIHR) supports the research most likely to bring benefits to patients and to the NHS. High quality proposals for support into any aspect of human health are always welcome. The NIHR Health Technology Assessment programme in particular is keen to receive suggestions for research topics in areas of market failure. These can be sent through the open access form at: www.hta.ac.uk/suggest/index.shtml.

To date, no application has been received regarding low dose naltrexone. Any application would be considered on its merits.

Yet more justification, if any were needed that the UK NHS needs to seriously look into widespread use of Low Dose Naltrexone (LDN) as a low cost treatment for Crohn's Disease, Ulcerative Colitis and the any other conditions it has been used successfully to reat including other auto-immune disease, cancers, autism and HIV/AIDs.

At £12,500 per patient per year for Infliximab versus £300 per year for Low Dose Naltrexone, it is the proverbial 'no-brainer' for the NHS to do whatever it takes to convince consultants and GPs that LDN is a suitable drug choice for IBD.

The government and public bodies need to cease dithering on this and take action now so that IBD sufferers need not suffer unnecessarily.

Last Saturday volunteers, patients and their friends were out on streets around the UK collecting signatures for the Number 10 Downing Street petition to encourage the UK NHS to fund trials of Low Dose Naltrexone so that every NHS patient has the option of Low Dose Naltrexone treatment on the NHS.

The day was a big success, around 3000 signatures have been collected, which when the site is updated will bring the total to around 7400 signatures.

As things stand at the moment, the petition is number 5 in the Health category and should move to 4 within the week.
In the overall list of all petitions it is number 24 and should be in the top 20 by the end of the week.

So, if you are a UK citizen and have yet to sign up, please do so now. If you have already signed up, please encourage at least 10 of your friends to sign up now and make this petition one that the UK Government cannot ignore.

The recent apology by Gordon Brown for Alan Turing's treatment shows that these petitions do count, so make your voice heard now. Background (Preamble):

I was diagnosed as being HIV+ in 1987: I participated in the Ozone trial in
the Philippines in 1993 while practicing various alternative and natural
therapies and am 60 years of age in Sept. 2009 and am healthy and well.

I pursued further Ozone treatment in 1999 in Kenya with the scientist who
developed that particular method of delivery called Polyatomic Apheresis -
The late Basil Wainwright - and returned to Australia early 2000. I
discovered in that year a seeming decrease in my CD4 cells and a
concomitant decrease in my platelets so I went on medication then and
remained fairly steadily on it for some time - four years -with a couple of
intermittent breaks from it due to the development of small lumps in my
sides (abdominal area). In all of this time my cd4's have been only between
200 and 300: however, after being on LDN for some three months - from round
September 2008 - they showed the most significant increase I have seen
since the Ozone trial in 1993.....CD4' s were 350 and CD8's were 1096 with a
non detectable viral load.

I had a few weeks off around the Christmas and have now passed another
three/four month period taking 4.5 mg every night. My latest results at May
2009 are ... non detectable viral load and CD4 count of 293. The present
cost of this medication at the lower end of the scale is $100+.00 every
three months. I am receipt of a disability pension. I have an interest in
drawing official attention to this medication and having it listed on the
pharmaceutical benefits listings.

Petition:
We petition the Australian government to make funds available for trials
and research for LDN to achieve a licence and public availability. This
medication must be added to the pharmaceuticals benefits listings.

We petition the Australian government to make funding available, either
directly through taxes or indirectly by directing research bodies to
provide funding, to undergo trials for the generic drug, Low Dose
Naltrexone (LDN).

Primarily this is to be done for Cancer since there lies the biggest
savings of life and economy, but then also for the many other diseases,
such as MS, Crohns disease, Fibromyalgia, Rheumatoid Arthritis, HIV/Aids
and many more.

Alternatively, the drug should be made available on the basis of it's
widespread use and the widespread positive reports of the many doctors
around the world who are currently using it. The evidence about this drug
has overtaken the trials process already and as such, we ask also that this
drug be made available as front line treatment, now. The research should
then be performed to confirm the drug and resolve the many unanswered
questions surrounding it.

To sumarise, we ask for the funding to be provided and the drug to be made
freely available to those who request it. The drug has been proven safe at
high dose usage so the safety trials are done, and we know of no drugs
which become unsafe at low dose when safe at high ones. In 25 years, the
drug industry has ignored this drug due to lack of return on investment, so
we believe that only government led intervention can break this scandalous
apathy. LDN now, save our sick, save our economy.

LDN Research Summary - Dr Tom Gilhooly, Essential Health Clinic, Glasgow

Although Low Dose Naltrexone (LDN) is just 1/50th of the standard dose of Naltrexone, a 30 year old drug with a good safety record, published research and science research is needed in order for UK NHS GPs and NHS clinicials in general to feel comfortable with prescribing LDN.

Summary of research -

Initial notable research - Penn State - A human study on Crohn's Disease patients showed great results. NOTE - With much larger doses than Dr T G is using.

Today - Dr Tom Gilhooly says we are 9 10ths of the way towards funding for the first trial in UK using LDN on Multiple Sclerosis (MS) patients (120 patients).

The patent protection for Naltrexone is long gone - So, it is unlikely there will be any studies carried out by any pharmaceutical company.

Therefore, University or - Charity funding is required

Hopefully government science funding in the UK will be available.

Again - Publication is required (Hence clinical use in Dr T G's clinic has not been written up - Not seen as relevant by most professionals)

Issue : Institutional resistance - Charities are wary of LDN

Accredited Funding needed - There is only 1 such MS charity, but they are not keen on LDN

Most research has concentrate on relatively large doses of 3 - 4 mg - Dr T G's clinic stat on 1mg and work up to optimum dose which is often not much more than 1 or even use 1mg -Therefore there is a need to educate researchers on dosage.

Short duration studies - 2 - 4 years needed for LDN. For use in cancer, 5 years - therefore more longer studies are more expensive, so need shorter cheaper studies with demonstrable results.

LDN in Cancer , MS - Dr Ian Zagon - Has been publishing results for 28 yrs. Has done multiple studies in animals (244 publications)

First in 1981 - Nalexone and cancer in mice.

Subsequently Dr Bernard Bihari became interested. He did lots of work but often unpublished. His work on HIV was refused publication by many journals so went unpublished.

Dr Ian Zagon - Found that opiate growth factor was increased by LDN and this seemed to naturally inhibit tumour growth in animals.

Used Experimental Allergic Encephalomyelitis (EAE) - This was induced in animals and treated wtih LDN

MS Society of America has funded research into this which will be published soon.

Dr I Z's MS study was presented at the September 2008 Montreal World Congress on MS.

Finding - High dose Naltrexone worsens EAE and makes humans feel worse.

Therefore, low dosages required and it is important to take once per day - So the effect us for a short duration.

Human Studies :

Fibromyalgia (FM) - Paper published last week.

There are 250,000 FM sufferers in the UK alone

Stanford - 10 FM patient study due to report in 2010.

Dr T G - Essential Health Clinic, Glasgow - Clinical findings -

Have been using LDN with FM for 6 months, much better than previous treatment - 80% improvement vs 20% with other approaches, e.g. Nutrition, lifestyle improvement.

Effect is very dosage dependant.

All studies used 4.5mg, TG - mostly 1 - 2 some people have trouble taking more than 1.

Penn State, Crohn's - Still the most impressive result.

Done by Professor Jill Smith, Penn State University.

17 patients over 12 weeks pilot study- Used 4.5 mg LDN (Again, note this is high compared with Dr T G's clinical experience).

Very rigourous - Close examination of colon.

Findings -

67% of patients went into remission.

89% responded positively in some way.

Dr T G's, EHC clinical findings - 3 patients , 2 with severe Crohn's - both entered remission with 1mg quite quickly. (NOTE - I am the 4th patient but I'm new! LOL)

One of Dr T G's NHS patients had bloody diarrhea so severe they had to have a blood transfusion. This patient is now in remission for over a year. The patient has actually stopped LDN as they didn't like to take drugs (!) but has maintained remission.

IBS Study- Kariv et al, Tel Aviv - 47 patients with 0.5mg (Very low dose) - Saw 76% improvement and increase in reduction in pain-free days.

Another study - 600 patients double-blind, randomised -

0.5 mg (Called PTI 901)

2 months of improvement, but no further improvement after 3 months.

Seems like too low a dose, so IBS not proven really.

Dr T G's EHC Clinical experience - LDN is the best MS treatment.

Treated 40 primary progressive MS patients (Worst case scenario) over 6 months

Only 1 patient out of the 40 experienced any progression in their symptoms.

There was a significant reduction in spasticity found.

Cree et al - University of California at San Francisco

80 patients for 8 weeks, randomised controller trial using 4.5 mg

Significant improvement using MS Quality of Life Index.

Pain, mental health and cognition all improved significantly.

NOTE - Side Effects - No major adverse effects have been found in any of the studies done so far anywhere.

Therefore - There is no risk in prescribing LDN.

In Summary :

There have been numerous publications of results of using LDN.

LDN has been tested in use for a diverse range of medical conditions

LDN Seems to be useful for many conditions involving the immune system

LDN A Personal Experience - Linda Elsegood of The LDN Research Trust, European LDN Conference 2009, Glasgow.

Linda has had MS for 20 yrs. Has been on LDN 5.5 yrs.

Today she appears totally symptom free.

Before LDN she has very limited mobility, cognitive issues in communicating and severe muscular problems.

In December 2003 she started taking LDN

She saw a dramatic improvement in 3 weeks - fatigue, sight, bladder - all improved.

Subsequently Linda set-up The LDN Research Trust (LDNRC) as a charity.

All work is voluntary work, just the running costs are paid from donations.

So far they have raised £22K towards a clinical trial in the UK.

LDNRC's aim - Get LDN on the UK NHS for everyone.

There are 4200+ registered LDN users with the LDNRC.

3000 are in the UK

90% have MS, others have Crohn's, ME, Parkinson's, Cancer and various other conditions.

Citizens of the US and other countries are welcome to register

Linda's current condition - She finds that getting an infection or virus brings on her MS symptoms (NOTE - Some people say they don't get infections or viruses at all while on LDN). But she finds the symptoms subside once the infection or virus is gone.

Low Dose Naltrexone (LDN) on the NHS

Surprisingly enough, this blog is aimed at helping to get Low Dose Naltrexone (LDN) available via prescription in the United Kingdom's National Health Service (NHS).

wikipedia has a short page on LDN (Surprisingly un-informative, the lowdosenaltrexone.org link above is more useful).

The LDN Research Trust is a UK charity which aims to increase awareness of LDN and it's uses and to raise funds for clinical trials to provide evidence of it's efficacy in treating auto-immune diseases and other conditions.

Dr Tom Gilhooly and his partner pharmacy, Dicksons's has been instrumental in publicising LDN in Scotland the UK generally via his clinic in Rutherglen, Glasgow.

Dr Gilhooly organised an EU conference on LDN in Glasgow which has just been held.

The next one will also be held in Glasgow in 2010, and will be over 2 days, 1 for clinicians and researchers and one open the public also.

I attended this conference and will be providing some notes on the content of the presentations given at it in this blog in subsequent postings.
